Correlation between platelet count and outcome of chronic HCV patients treated with direct-acting antivirals

Background: Egypt has the highest prevalence of chronic hepatitis C virus (HCV) infection. The<br />direct acting anti-virus (DAAs) are available, with a reported 95% sustained virological response<br />after treatment for 12 weeks (12w-SVR).<br />Objectives: The current study aimed to assess the correlation between platelet count and the<br />DAAs therapy outcome in chronic HCV (CHC) patients.<br />Patients and methods: three hundred CHC patients, Child–Pugh grade A, both naïve and<br />treatment experienced patients were enrolled in this study, from outpatient Clinic, Department of<br />Gastroenterology, Hepatology and Tropical medicine, Qena university hospital, Qena, Egypt,<br />treated with DAAs for 12 weeks, either dual or triple therapy, according to criteria recommended<br />by the national committee for chronic viral hepatitis (NCCVH). Patients categorized into three<br />groups: (1) Group I (non-thrombocytopenic group): included 100 CHC patients with platelet count<br />≥ 150 (109/L); (2) Group II (mild thrombocytopenic group): included 100 CHC patients with<br />platelet count 100-149 (109/L); (3) Group III (moderate thrombocytopenia): included 100 CHC<br />patients with platelet count 50–99 (109/L).<br />Results: The Overall CHC patient's mean age were (48.2 ± 11), 226 (75.33%) were males and 74<br />(24.67%) were females. 97.6% (293/300)of CHC patients attainedSVR; 97 % in (Group I), and 98<br />% in both (Group II), and (Group III), after 12 weeks DAAs therapy with no significant difference<br />between groups.<br />Conclusion: both DAAs treatment modalities were efficient in the eradication of HCV; however,<br />thrombocytopenia in CHC patients does not affect the DAAs therapy outcome.

Objectives: determine the prevalence of acne vulgaris among teen-agers in Qena city, determine the<br />risk factors of acne vulgaris, and determine the effect of acne vulgaris on the quality of life among<br />teen-agers in Qena city.<br />Patient and methods: The study includes 200 students in Qena secondary schools, the students<br />were asked to fill in 2 questionnaires, the first include personal date and the other include is CADI.<br />Results: the prevalence of acne vulgaris in Qena secondary school students was 58, acne vulgaris<br />was more prevalent in males (53.5%) than females (46.5%). In addition, We found 5 risk factors<br />associated with AV, the most contributing risk factor for AV of these 5 risk factors was stress (AOR<br />= 18.85, 95% CI 6.313-56.281), regular eating of spicy food (AOR = 13.364, 95% CI 4.525-39.465)<br />and the least contributing risk factor was eating fast food. In our study CADI score was significantly<br />related to the disease grade, being maximum among those with severe grade, followed by moderate<br />and lastly mild disease grade.<br />Conclusion: Acne vulgaris is prevalent among Qena teen-agers, stress and dietary factors are<br />important risk factors of acne, CADI score was significantly related to the disease grade.

Idiopathic Sudden Sensorineural Hearing Loss(ISSNHL) is a an irritating problem that<br />continues to pose a diagnostic and therapeutic enigma for the otologist. It is most<br />commonly defined as sensorineural hearing loss of 30dB or greater over at least three<br />contiguous audiometric frequencies occurring within a 72-hr period. It is not one of the<br />more common causes of HL, disproportionate interest in ISSNHL exists most likely<br />because it is one of the few reversible (sensorineural) hearing losses encountered by<br />clinicians. Intratympanic steroid injection is increasingly used in the management of<br />ISSNHL as it offers the potential for directed therapy of a high concentration to the inner<br />ear with avoidance of systemic side effects. It also can improve hearing function in<br />patients with ISSNHL after the failure of systemic steroid treatment.

Background: Tubal dysfunction is responsible for approximately 30% of infertility cases. Tests to<br />determine if tubes are open and undamaged are an important part of the infertility workup.<br />Objectives: The aim of this study was to find out the predictive values of bubble test during office<br />hysteroscopy in assessment of tubal patency. .<br />Patients and methods: This study included 100 cases underwent for both hysteroscopy and laparoscopy at<br />the same setting in department of obstetrics and gynecology , Qena University hospital, South Valley<br />University, Egypt.<br />Result: In our study 100 women were included , 74 presented by primary infertility and 26 presented by<br />secondary infertility..In our study using bubble test for tubal patency as primary outcome , the results<br />showed that bilateral tubal patency represented 71 %, unilateral tubal block represented 11% and bilateral<br />tubal block 18%.And the results of laparoscopy with MB injection for tubal patency assessment as<br />secondary outcome , showed that bilateral tubal patency represented 75%, unilateral tubal patency 13 % and<br />bilateral tubal block 12%.so , we figured out that Evaluation of Diagnostic Performance of bubble test in<br />hysteroscopy right in sensitivity of 87.9% specificity of 54.5% positive predictive value of 92.1%, negative<br />predictive value of 42.9% with diagnostic accuracy of 83.1%<br />Conclusion: Bubble test had a good predictive value in diagnosis of tubal patency through office<br />hysteroscopy.

Background: Advances in anesthetic induction agents and airway management have contributed to<br />the success of day case surgeries. An ideal day-case anesthetic agent should have rapid smooth<br />induction; provide optimum surgical conditions with rapid recovery and minimal side effects.<br />Objectives: Comparing isoflurane and sevoflurane as maintenance anaesthetic agents in day case<br />surgeries.<br />Patients and methods:This study was done on 100 patients scheduled for operations under general<br />anesthesia divided in two groups of maintenance anaesthesia (isoflurane and sevoflurane) 50<br />patients in each.<br />Results: MAP was with more decrease in group B (75.8±7.2) than group A (90.2±8.8). LMA<br />hyper- reactivity score was with more decrease in mild (20%) and moderate (6%) in group A than<br />group B, mild (46%) and moderate (8%). LMA removal (min) was statistically significant between<br />two groups A (8.8±1.2) B (5.1±.8). Emergence was statistically significant with (0% of cases) and<br />(24% of cases) for groups A and B respectively. PONV score, was statistically significant with<br />negative (88%, 98%) and mild (12 %, 2%) for groups A and B respectively. discharge time, was<br />statistically significant with (21.1±8.3 hours, 9.4±2.5 hours) for groups A and B respectively.<br />Conclusions:Sevoflurane and isoflurane are suitable for day care anaesthesia.

Background: Various analgesic modalities have been used for postoperative analgesia in patients<br />undergoing inguinal hernia surgery.In this randomized clinical trial, we have compared the analgesic<br />efficacy of transverses abdominis plane (TAP) block with that of ilioinguinal/ iliohypogastric (II-IH) nerve<br />block and quadrates lamborum(QL) block in patients undergoing unilateral open inguinal hernia repair.<br />Objectives: comparison of the efficacy of either of the three types of regional anesthetic blocks , the<br />ultrasound –guided TAP block , ultrasound-guided Quadratus Lamborum block and ultrasound- guided<br />ilioinguinal and iliohypogastric nerve block in quality and duration of postoperative pain control.<br />Patients and methods:This is a cross-sectional, hospital based study carried out on 90 cases undergoing<br />inguinal hernia repair under general anesthesia using either of the three types of block in each group; the<br />ultrasound–guided TAP block , ultrasound- guided ilioinguinal and iliohypogastric nerve block and<br />ultrasound-guided Quadratus Lamborum block nerve All cases enrolled fromgeneralsurgerydepartment and<br />seen at the pre operative holding area before going to the operative theatre of Qena university hospital<br />between August 2018 to August 2019 .<br />Result(s):VAS were lower in the IINB group and QLB group compared to the TAP block group both at<br />rest and during cough. The difference in VS score was statistically significant various times postoperative.<br />Time to first analgesic request was delayed in IIIH nerve block Group(7.3±1.3 ) hours than TAP block<br />group (5.9±1.3)hours (P value .008)and time to first analgesic request was delayed in QL block Group < br />(10.3±3.9) hours than TAP block group (5.9±1.3)hours (P value 0.001) .15 patients (50%) in TAP block<br />group,10 patient (33.3%) in IINB group and 4 patients (13.3%)required morphine 24 hours postoperative .<br />Conclusion:This study demonstrated that compared to TAP block II-IH nerve block and QL block provides<br />better pain control after open repair of inguinal hernia when all blocks were administered under US<br />guidance.

Background:Amino acids can elevate sperm count, motility, and morphology. Increasing the availability<br />of these amino acids can help to improve male fertility certain foods can help to improve sperm<br />parameters. This is due to the high concentration of specific amino acids. There are many amino acids<br />involved in the production, development and maintenance of sperm.<br />Aims:assessment of seminal plasma free amino acids among fertile men.<br />Patients and methods: this study done on 50 fertile menassessment of free amino acids in their seminal<br />plasma, Semen analyses were done and amino acids measured in seminal plasma using H.P.L C.<br />Results:The median and range of their semen analysis parameters were :volume 2(3-3),liquefaction<br />time 10(10-15), sperm concentration 60(50-75)million / ml ,total sperm motility80(80-80) %, abnormal<br />sperm35(30-40),viability 67.5(60-80)%the range of seminal plasma leucine was(1.68-1.87),isoleucine<br />(1.22-1.31),valine(0.33-0.53)tyrosine, (1.11-1.22),phenylalanine (0.79-0.89) ,histidine (1.28-1.51)<br />,Tryptophan (0.26-0.36).<br />Conclusions:Assessment of seminal plasma amino acids is very helpful in evaluation of male fertility.

Background: Parasitic diseases continue to be a major cause of morbidity and mortality. More than<br />three billion people are infected worldwide, mainly in the developing countries. Children are easy prey<br />to parasites. Poverty, illiteracy, poor hygiene, lack of access to potable water, poor public health<br />infrastructure, hot and humid tropical climate are the usual factors associated with intestinal parasitic<br />infections. Parasitic infections, particularly intestinal helminthes, cause hundreds of thousands of<br />avoidable deaths each year<br />Objective: To Assessment of the prevalence of parasitic helminthes infection among Qena governorate<br />residents.<br />Patients and method(s): Across sectional study was carried out on 316 patients, 5-50 years attended<br />the outpatient clinics of Qena Faculty of Medicine, South Valley University, between August 2017 and<br />August 2018. Stool samples were collected and examined by Direct wet mount and Concentration<br />techniques including sedimentation using saline and formol ethyl acetate.<br />Result(s):In the studied cases, intestinal helminthic parasitic infections were found in 40 (12.5%) cases<br />out of 316 patients. Positive cases were 30 male patients (75%), while 10 female patients (25%), 27<br />(67.5%) patient out of 197 aged from 5-19 years, 0 (0%) patients out of 70 from 20-34 years and 13<br />(32.5%) out of 49patientfrom 35-50 years, 10 of patients from urban houses (25%) and 30 of patients<br />from rural houses (75 %),19 (47.5%) were present with abdominal pain , 9 (22.5%) with diarrhea and<br />12 (30%) with perianal itching<br />Conclusion: Qena governorate suffer from intestinal helminthic parasitic infection mainly in rural<br />areas in children and young adults from 5-19 years old.

Background: Optical Coherence Tomography is the key tool for assessment of the retinal nerve fiber<br />layer (RNFL) thickness which is occasionally altered in myopic patients.<br />Aim of study: To assess the effect of different degrees of myopia on the thickness of retinal nerve<br />fiber layer by the use of spectral domain optical coherence tomography .<br />Patients and Method(s): Our study included examination of 40 myopic patients and 40 emmetropic<br />persons.Retinal nerve fiber layer thickness was measured by the use of Optical Coherence<br />Tomography.<br />Results: Evaluation of RNFL thickness at the different sectors revealed that the thickness at the<br />superior sector is of 102.13 ± 24.91μm for the myopics and of 124.5 ± 15.58 μm for the emmetropes.<br />At the inferior sector the thickness was of 114.08 ± 25.15μm for the myopesanf of 134.63 ± 19.08<br />μm for the emmetropes ,at the nasal sector the thickness was of 71.9 ± 20.62 μm for the myopes and<br />of 81.48 ± 9.45 μm for the emmetropes, at the temporal sector the thickness was of 75.25 ± 23.4 μm<br />for the myopes and of 74.78 ± 13.42 μm for the emmetropes .<br />Conclusion: Different grades of myopia affects the RNFL thickness by different degrees with the<br />thinnest one goes to the high myopia. So, High myopia should be considered in the interpretation of<br />OCT data because of thinning of RNFL thickness, and normative database corrected for refractive error<br />and axial length should be incorporated.

Background: In mitral valve prolapse is associated with severe mitral regurgitation, mitral valve repair<br />or surgical replacement may be necessary. In Mitral valve replacement, some surgeon use continuous<br />suture, others use interrupted suture technique.<br />Aim of the work: to compare efficacy of semi-continuous technique & interrupted suture of prosthestic<br />valve replacement in mitral valve replacement.<br />Patients and Methods: it was a prospective randomized comparative study done at the Cardiothoracic<br />Surgery Department, Qena University Hospitals, South Valley University, Egypt between April 2016 to<br />April 2018. It included 31 patients with rheumatic mitral valve disease whatever the cause. They were<br />divided into group1: patients underwent semi-continuous technique for MVR (18 cases) and group2:<br />patients underwent interrupted technique (13 cases).All of patients were subjected to preoperative data:<br />complete history, general examination including general condition of the patient and vital signs,<br />investigations including routine laboratory investigations, especially complete blood count, chest X ray<br />(CXR), 12 lead Electrocardiography (ECG), echocardiography and coronary angiography if patient<br />more than 40 years old, operative data including total bypass time, aortic CCT, number of sutures, and<br />postoperative outcome including ICU stage and inotrope use, postoperative complications, pulmonary<br />hypertension and echo findings as immediate postoperative and echo after 1 and 3 months<br />postoperative.<br />Results: We found that the mean total bypass time was significantly higher among interrupted group < br />compared to semicontinuous one.<br />Conclusion: Semicontinuous sutures showed significantly better results than interrupted ones,<br />especially regarding the total bypass time
