Effect of the type of urinary incontinence in the Outcome of laser Therapy for Treatment of Female Stress Urinary Incontinence

Background:Female stress urinary incontinence (SUI) is common lower urinary tract symptom. It affect many<br />women especially middle and elderlywomen and about 40% of them have mixed Urinary incontinence<br />(MUI).There are surgical and non-surgical options for treatment of stress urinary incontinence. Laser<br />therapy is seemed to be a promising minimally invasive option for treatment of female SUI.<br />Objective:To determine the effect of the type of urinary incontinence in laser therapy outcomes.<br />Patients and method(s): A prospective non-randomized study for female patients with SUI who were referred<br />to the Urology Department, Sohag University Hospital from March 2017 to June 2018. The study was<br />approved from Sohag University ethics committee. Patients were treated by the clinical protocol for SUI<br />with non-ablative 2940 nm Er: YAG laser. Patients were evaluated at the baseline and every 3 months for 1<br />year by Validated International Consultation on Incontinence Questionnaire – Urinary Incontinence Short<br />Form (ICIQ-UI SF) Arabic version and the number of daily pads usage .The patients were classified<br />according to their type into a pure SUI group and MUI group.<br />Result(s):Our results included 32female patients. 20 patients (63 %) had pure SUI while 12 patients (37%) had<br />mixed incontinence. the median duration of symptoms 3 years. There was a significant reduction in daily<br />pads usage and ICIQ-UISF score (p value<0001) for both groups with more significant for pure SUI group.<br />No serious side effects were reported.<br />Conclusion:There was a statistically significant effect of the type of UI on laser therapy outcomes with better<br />results related to pure SUI.On the other hand, No effect of type ofurinary incontinence on number of laser<br />sessions needed by patients to achieve improvement of their incontinence.<br /><br />

Background: Posterior capsule opacification is the most common complication of cataract surgery<br />and results from the proliferation and migration of residual lenticular epithelial cells. Opacification<br />may be diminished by atraumatic surgery; complete cleaning of cortex residues, polishing of both<br />anterior and posterior capsules, clinical, pathological and experimental studies have shown that use<br />of hydrodissection, the continuous curvilinear capsulorhexis or the use of specific intraocular lens<br />(IOL) designs. The overall incidence of posterior capsule opacification (PCO) approaches 50% at 5<br />years following cataract surgery and disproportionately affects younger patients due to higher cell<br />proliferation rates. Capsular-fixated, one-piece all-polymethylmethacrylate PC-IOLs with a Cshaped<br />loop configuration and a posterior convexity of the optic are effective in reducing PCO.<br />Objectives:To evaluate the influence of size and shape of neodymium: yttrium aluminum- Garnet<br />(Nd:YAG) laser capsulotomy on visual acuity and refraction.<br />Patients and method(s): A prospective study was performed in department of ophthalmology at<br />Qena university hospital between September 2016 and September 2017. Informed consent was<br />obtained from each patient before the study. A South Valley University institutional ethics<br />committee approval was obtained.<br />In this study 60 pseudophakic eyes of 58 patients of age range (30-70years) were included. All of<br />the patients had undergone non complicated cataract surgery (phacoemulsification (48eyes) or<br />extracapsular cataract extraction (12 eyes)).<br />Result(s): All patients had significant vision loss and hazy fundus appearance due to posterior<br />capsular opacification. There was significant improvement in VA in the four groups, with<br />statistically significant improvement effect of cruciate shape capsulotomy with size >3.5mm on<br />BCVA.<br />Conclusion:The cruciate 0shape with size more than 3.5 mm capsulotomy openings show better<br />improvement in BCVA than the other groups. Spherical equivalent show slight improvement in all<br />groups which was insignificant.

Background: Diabetes Mellitus (DM) is a metabolic disorder, caused by impaired carbohydrate<br />metabolism makes many changes in the blood vessels affecting the response of endothelium and the<br />smooth muscles. Polyol pathway activation, protein non-enzymatic glycosylation and increased<br />ROSs(reactive oxidative species) play an important role in the complications of diabetes.Ozone<br />activates glucose metabolism that results in increasing content of 2,3diphosphoglycerate in erythrocytes<br />which provides better oxygen supply into the tissues. Patients with diabetes mellitus have the so called<br />glycosylated hemoglobin forming very strong bonds with oxygen, thus, inducing hypoxia and<br />determining the severity of the disease. That is why hypoxia control with the help of ozone therapy is<br />of the key importance in the course of treatment.<br />Conclusion: Ozone therapy appears to be an effective method for DM treatment. The reason is inozone<br />mechanisms when it can perform a number of processes, which provide its positive effect.

Background: Sub-endometrial junction zone (JZ) plays an important role in most of reproductive<br />functions. Objective: To find out the effect of the sub-endometrial JZ thickness assessment by 3-D transvaginal<br />ultrasound (TVUS) on intracytoplasmic sperm injection (ICSI) outcomes in patients with<br />unexplained recurrent implantation failure (RIF). Setting: ART Unit of Obstetrics and Gynecology<br />Department, Faculty of Medicine, South Valley University, Qena, Egypt. Duration: From April 2016 to<br />October 2018. Study Design: A prospective observational study. Methds: Fifty couples with history of<br />unexplained RIF in previous ICSI cycles and prepared for another ICSI cycle (group I) and fifty couples<br />with unexplained infertility prepared for ICSI for the first time (group II) had been included in this study.<br />At time of ovum pick up, 3-DTVUS was done for all cases in both groups for assessment of sub<br />endometrial junction zone thickness and correlated with ICSI outcome. Results:There were statistically<br />significant differences be- tween group I and group II in JZ thickness in the 3 uterine regions (fundus,<br />anterior and posterior walls) with p < 0.001, but there were mildly statistically significant differences<br />between both groups in chemical and clinical preg- nancy rates with p < 0.01. Conclusions: The<br />thickness of JZ in patients with history of unexplained RIF was higher than those with unexplained infertility<br />scheduled for ICSI. The JZ thickness was inversely correlated with increased embryo<br />implantation rates in ICSI procedures, the thinner the JZ thickness was associated with higher pregnancy<br />rates (both chemical and clinical pregnancy rates).

Background: Immunodeficiency disorders involve malfunction of the immune system,<br />resulting in infections that develop and recur more frequently, are more severe, and last longer<br />than usual, Impair the immune system's ability to defend the body against foreign or abnormal<br />cells that invade or attack it (such as bacteria, viruses, fungi, and cancer cells). As a result,<br />unusual bacterial, viral, or fungal infections or lymphomas or other cancers may develop.<br />Patients and methods: A study to detect blood stream infections was done using 100 blood<br />cultures obtained from 100 immunocompromised children.<br />Results and conclusions: The most commonly isolated are bacterial organism (40 %)the most<br />common bacterial isolates are klebsiella species (17 | 40 ,42.5%). Estimation of DNA damage<br />was done for 40 patients and 10 apparently healthy children as control subjects. The DNA<br />damage was detected has took the smear shape on gel electrophoresis. This was found in (12|40,<br />30%), (P value < 0.001) with significant difference.

Background: Chronic Kidney Disease (CKD) is a globally increasing condition that almost always finally leads to renal replacement therapy. Intestinal parasitic infections are amongst the widest spread of all chronic human infections worldwide. Materials and methods: 50 stool samples were collected from patients who suffer from Chronic Kidney Disease from Qena Governorate Hospitals and 50 stool samples from healthy control group. The fecal samples were examined macroscopically to determine the consistency, presence of blood or mucus and presence of parasites. Results: Intestinal protozoal infections were detected in 33 out of 50 patients in the CKD group (66%). Entamebahistolytica infection was detected in 7 out of 50 patients in the CKD group representing 14% of the total infections. Conclusions: Entamoebahistolyticainfection in the CKD are within the universal levels.

Background: To date, there is a lack in the published literature that reported the risk<br />factors for diarrhea for under-five children in Egypt.<br />Objectives: we conducted the present cross-sectional study to investigate the role of a<br />number of possible risk factors for diarrhea among under-five children in Egypt.<br />Patients and methods: The present study was a cross-sectional study that carried out<br />from February 2017 to August 2017 at the pediatric outpatient clinics of El Amal<br />Maternal and Child Health in Quasar city.<br />Results and conclusion: A statistically significant higher proportion of patients who did<br />not exclusively breastfeed were more likely to have more than four episodes of diarrhea<br />per day(p =0.026). Similarly, diarrheal episodes more than four motions per day was<br />significantly more prevalent among children who consumed unstored cooked food (p < br />=0.1) or did not receive measles vaccine (p =0.04). Notably, children of mother who<br />knew the benefits of breastfeeding were less likely to suffer from diarrheal episodes (p < br /><0.001). However, diarrheal episodes per day did not have significant associations with<br />age (p =0.656), sibling number (p =0.645), number of rooms within the house (p= 0.145),<br />and number of people number living in the house (p =0.721).

Background:The central purpose ofantenatal care is to screen for and identify high risk pregnancies as early<br />as possible from a general population of pregnant women, and then provide appropriate skilled care for<br />women with high risk pregnancies while continuing to give adequate care for the women with low risk<br />pregnancies. This “risk approach” is a managerial tool for improving maternal and child health care.<br />Objective:To evaluate the impact of antenatal care for pregnant women with high risk pregnancy.<br />Patients and method(s): This is a prospective cohort study of a group of pregnant women with high risk<br />pregnancy. Detailed history, examination, and screening investigations were done throughout pregnancy.<br />Setting:pregnantwomen’s with high risk outpatient clinics and emergency unit, Qena university hospital,<br />Egypt<br />Result(s):The study included 120 women with high risk pregnancy. There was 16.7% incidence of first<br />trimester complications (hyperemesis& threatened miscarriage). Most of women suffered from second and<br />third trimester complications (80.8%) as preeclampsia in 20.8%, Gestational diabetes in 15%, PROM<br />in13.3%, and preterm labour in 9.2%. fetal complications is seen in 35.8%.<br />Conclusion: There was significant relationship between number of visits and the occurrence of<br />complications during the second and the third trimester and during the post-delivery period.

Background:In end-stage patients when all management options have been used, will often<br />develop compromise of their airways as the cancer continues to progress. Endobronchial<br />therapy options may help to relieve some of their symptoms, allowing improvement in their<br />shortness of breath as they go home in combination with other palliative therapies.<br />Objective: To compare the safety; efficacy; and clinical outcome of endobronchialcryotherapy<br />combined with chemotherapy and/ or radiotherapy versus chemo-radiotherapy alone on patients<br />with Non-small cell lung cancer.<br />Patients and method(s): A prospective randomized clinical trial was carried out on 60 patients,<br />diagnosed as bronchogenic carcinoma non-small cell lung cancer type (NSCLC). The patients<br />attended the hospital in the chest department, Qena Faculty of Medicine, South Valley<br />University. Assessment of the patients was done and randomly assigned into two<br />groups.Group1, Include 30 patients subjected to chemotherapy, radiotherapy and<br />endobronchialcryotherapy.Group2 ,Include include 30 patients subjected to chemotherapy and<br />radiotherapy alone.<br />Result(s): There was significant response meat in symptomatology in Group (1) cryotherapy<br />with regard to cough (P value .000) , dyspnea (P value .000) and haemoptysis (P value .000) .<br />Also, from the functional point of view, in group 1 there were significant improvement to<br />cryotherapy was achieved in FVC(P value .000) , FEV1 (P value .000) , PEFR (P value .000)<br />, and 6MWT (P value .000) . However, there were no significant difference in group 2 between<br />the incidence of improvement in FVC , FEV1 , PEFR , and 6MWT.<br />Conclusion:There wasbenifical effect of combination of endobronchialcryotherapy to patients<br />with lung cancer plus chemo and/ or radiotherapy in the form of improved symptoms and lung<br />functions.

Aim of the work: to evaluate the effect of intravenous dexmedetomidine infusion on patients<br />undergoing open heart surgery regarding stress response markers as plasma interleukin-6 and<br />plasma cortisol.<br />Patents and Methods: Fifty consecutive patients aged 18-70years, scheduled for elective open<br />heart surgery using CPB technique were enrolled in this study at the Cardiothoracic Surgery Unit,<br />Luxor international hospital, from July 2018 to May 2019. Patients randomly assigned using closed<br />envelope technique into two equal groups according to the drug infused intraoperatively: 1st group < br />(25 patients):- Dexmedetomidine group (Group D); received loading dose of intravenous<br />dexmedetomidine infusion of 1 μg/kg IV dissolved in 20ml normal saline over 10 minutes followed<br />by maintenance dose of 0.5 μg/kg/hr. till the end of surgery where 0.5 μg /kg/h was calculated for<br />each patient and dissolved in 12.5 ml normal saline 2nd group ( 25 patients):- placebo group (Group < br />P); receive 20ml intravenous infusion of normal saline 0.9 % over 10 minutes followed by<br />continuous infusion of 12.5 ml of normal saline till the end of surgery.<br />Results: Our study revealed that HR and MAP wasn’t significantly different between the two<br />groups at the baseline, but significantly lowered in Dex. group after induction, during CPB, post<br />CPB, and postoperative.Dexometomedine suppress the intraoperative and postoperative rise of IL-6<br />which was significantly higher in placebo group relative to Dex. group. Also dexometomedine has<br />effect on stress hormone as serum cortisol and serum glucose decease in Dex. group relative to<br />placebo group. None of studied cases revealed bradycardia or hypotention and there are no<br />significant difference between groups regarding side effect drowsiness, nausea and vomiting<br />Conclusion: This study demonstrated that the continuous administration of dexmedetomidine<br />during open heart surgery with CPB suppressed intraoperative and post-operative cytokine<br />secretion, and improved post-operative inflammatory response indices in the present study. Also it<br />stabilize blood pressure and heart rate and blunt cardiovascular response to CPB. These results<br />could be attributed to the anti-inflammatory effects of dexmedetomidine.
